Health Disparities Grant Program Spring Celebration for Colorado Grantees

The Home2Health project lives under the Health Disparities Grant Program (HDGP) which receives its funding though the Colorado state tobacco tax revenue. The program funds many grantees throughout the state of Colorado which all have a varied focus. The grant projects have focuses on community empowerment, food disparities, environmental health and housing needs.

We would like to extend the invite to this celebration to our community partners and to those that have followed our project since its infancy in 2019. The Home2Health project will wrap up this June so, please join us to see the work we have completed and to celebrate all the HDGP grantee successes.
